Notice of Corporate Dissolution and Solicitation of Claims

To All Known Creditors and Claimants Of:
Clear View Productions, Inc.

Dear Creditor and/or Claimant:

1. Please be advised that Clear View Productions, Inc. (the “Corporation”) has been dissolved, The effective date of dissolution was June 23, 2025.

2. Any claim against the Corporation must be sent in writing to the following mailing address:

3. Your claim must include

  1. The exact amount claimed;
  2. A description of the basis for the claim (e.g., invoice number, contract date, or nature of services provided)
  3. All supporting documentation (e.g., copies of unpaid invoices or contracts) You may fill in and mail a claim form, with any relevant supporting attachments, to satisfy this requirement. You can request a claim form with a fax to (312) 610-5744 or email to [email protected].

4. Deadline: The Corporation must receive your claim by May 31, 2026

5. STATUTORY BAR: NOTICE: YOUR CLAIM WILL BE BARRED IF IT IS NOT RECEIVED BY THE DEADLINE STATED ABOVE OR DOES NOT CONTAIN ADEQUATE SUPPORTING DOCUMENTATION. FILING A CLAIM DOES NOT GUARANTEE A DISTRIBUTION WILL BE MADE, BUT TO RECEIVE ANY DISTRIBUTION YOU MUST FILE A CLAIM.
